What is a Comorbid Condition?

A comorbid condition is one or more disorders or diseases co-occurring with a primary condition. In the context of attention deficit hyperactivity disorder (ADHD), this is often another mental health condition or neurodevelopmental disorder, such as:

  • Mood disorders
  • Anxiety disorders
  • Learning disabilities
  • Behavioural disorders

In children with ADHD, the rate of comorbid conditions is also high. Research indicates that 60% of children and adolescents with ADHD have at least one comorbid psychiatric disorder.

What are the Most Common ADHD Comorbidities?

Mood Disorders

People with ADHD frequently experience depression and mood swings, and are at significantly higher risk for developing chronic mood disorders, such as:

  • Major depressive disorder: A disorder characterised by persistent feelings of sadness and hopelessness. It can exacerbate ADHD-related difficulties with concentration and motivation.
  • Bipolar disorder: A mood disorder with alternating periods of mania and depression. Bipolar disorder can complicate ADHD diagnosis due to overlapping symptoms, such as impulsivity and hyperactivity.
  • Chronic depression: A long-term, low-grade form of depression. It can intensify feelings of inadequacy and affect daily functioning in individuals with ADHD.
  • Disruptive mood dysregulation disorder (DMDD): Common in children with ADHD, DMDD is characterised by recurrent emotional outbursts and persistent irritability.

Anxiety Disorders

In addition to mood disorders, anxiety disorders are also common in people with ADHD. Comorbid anxiety can come in many forms, including:

  • Generalised anxiety disorder: A condition marked by excessive, uncontrollable worry. It can amplify difficulties with focus and increase restlessness in ADHD individuals.
  • Obsessive-compulsive disorder: A disorder characterised by intrusive thoughts and repetitive behaviours. It may intensify organisational challenges and perfectionism when comorbid with ADHD.
  • Social anxiety disorder: A disorder characterised by an intense fear of social situations. Social anxiety in ADHD can exacerbate difficulties in social interactions and increase self-consciousness regarding ADHD symptoms.

Neurodevelopmental Disorders

Several neurodevelopmental disorders commonly co-occur with ADHD. Autism spectrum disorder, characterised by challenges in social interaction and communication, is common in children with ADHD. 

According to research, 9.8% of children with ADHD also have autism spectrum disorder. Autism can make it more difficult for people with ADHD to navigate social situations and process sensory information.

Learning disorders, which include difficulties in reading, writing, or math, can further complicate academic performance and self-esteem in students with ADHD. Many people with ADHD also have tic disorders, which are sudden, repetitive movements or vocalisations. Tic disorders can increase social discomfort and complicate symptom management when co-occurring with ADHD.

Behavioural Disorders

Behavioural disorders also frequently overlap with ADHD. Oppositional defiant disorder, characterised by persistent disobedience and hostile behaviour, can exacerbate difficulties with authority figures for children and adults with ADHD. Conduct disorder, which involves more severe patterns of violating rules and others' rights, can lead to more serious behavioural issues and social conflicts when comorbid with ADHD.

Personality Disorders

Certain personality disorders can also co-occur with ADHD. Borderline personality disorder, marked by unstable relationships, self-image issues, and emotional instability, can intensify emotional dysregulation and impulsivity in ADHD individuals. Antisocial personality disorder, characterised by disregard for others' rights and societal norms, may lead to more severe impulsive and risky behaviours when comorbid with ADHD.

Other Comorbidities

Some other comorbid conditions often seen in children and adults with ADHD include: 

  • Sleep disorders: People with ADHD may also have difficulties falling asleep or getting adequate sleep, exacerbating daytime fatigue and concentration problems.
  • Substance abuse disorders: Substance abuse is common in people with ADHD. Problematic use of alcohol or drugs can worsen impulsivity and complicate ADHD treatment.
  • Eating disorders: Disorders such as anorexia or bulimia often result from impulsivity and poor self-regulation in ADHD.
  • Internet gaming disorder: Excessive and problematic gaming habits are also common in people with ADHD. Compulsive gaming habits can make it more challenging for people with ADHD to manage time and fulfil daily responsibilities.

Impact of Comorbidities on ADHD

Comorbidities can significantly impact ADHD if left untreated. They often exacerbate ADHD symptoms, leading to more severe attention problems and heightened impulsivity. The interplay between ADHD and comorbid conditions can also create a cycle of worsening symptoms. For example, anxiety can increase ADHD symptoms, which in turn feed into anxiety. This makes early diagnosis and treatment crucial.

Treatment also becomes challenging as interventions effective for ADHD may be insufficient for treating comorbidities. For example, medication for ADHD may worsen another co-occurring condition. This makes accurate diagnosis of comorbid psychiatric disorders important, as it allows ADHD specialists to design treatment plans that consider all conditions.

How to Differentiate ADHD Symptoms From Comorbidity Symptoms
