What Concerta XL is

Concerta XL is a modified-release brand of methylphenidate, the most commonly prescribed ADHD medicine in the UK. It is designed to be taken once in the morning and to provide cover for up to twelve hours.

What makes it distinctive is the delivery system, known as OROS. The tablet has an outer coat containing 22 per cent of the dose, which is released immediately, and an inner core containing the remaining 78 per cent, which is pushed out gradually by an osmotic mechanism as water is drawn in.

The practical consequence is that the tablet shell passes through unchanged and can be seen in the stool. That is expected and does not mean the medicine has not been absorbed. It also means the tablet must be swallowed whole: crushing, splitting or chewing it destroys the mechanism and releases the full dose at once.

Who Concerta XL is prescribed for

Methylphenidate is licensed in the UK for children aged six and over as part of a wider treatment programme, and for adults. NICE guideline NG87 makes it the usual first choice for children and young people, and one of two first-line options for adults.

A twelve-hour option suits people whose day genuinely runs that long, and people for whom remembering a midday dose is unreliable. It is often the formulation chosen once titration on an immediate-release product has established roughly the right total dose.

Clinical context

Checks before starting

A specialist will check height, weight, pulse and blood pressure before starting, and ask about heart conditions in you and your family. These checks are repeated during treatment, and in children height and weight are tracked at every review.

Because the OROS shell does not deform, Concerta XL is generally avoided where there is a history of significant narrowing anywhere in the gastrointestinal tract.

Dosage and how it is taken

Concerta XL comes in several strengths and is taken once daily in the morning, with or without food. The dose is adjusted in steps at intervals of about a week, with a review at each stage.

It is swallowed whole with liquid. Some people are established on Concerta XL with a small immediate-release dose later in the day to extend cover, which is a specialist decision rather than something to arrange independently.

Prescribed by brand

The MHRA advises that modified-release methylphenidate is prescribed by brand name. Concerta XL, Medikinet XL and Equasym XL release the dose differently and are not interchangeable. If your supply changes brand, raise it rather than assuming it makes no difference.

How long Concerta XL lasts

Cover typically extends up to twelve hours from the morning dose. The 22 per cent immediate portion means something is usually noticeable within the first hour, and the remainder is delivered steadily rather than in a second peak.

In practice a typical day might start with the tablet at around seven in the morning, with cover holding through the working or school day and easing off during the evening. Some people notice a period as it wears off where tiredness or low mood is more apparent.

Duration varies between individuals. If cover consistently runs out well before the day does, that is worth reporting: it is often addressed by adjusting the dose or adding a small immediate-release dose rather than by changing medicine.

How Concerta XL compares

Same drug as Ritalin, released across a very different curve.

Brand Release Duration Doses per day
Concerta XL Modified, OROS 22/78 Up to 12 hours One
Medikinet XL Modified, 50/50 About 8 hours One
Equasym XL Modified, 30/70 About 8 hours One
Ritalin Immediate 3 to 4 hours Two or three

The numbers describe how the dose is split between immediate and gradual release. Concerta XL front-loads least, at 22 per cent, and lasts longest. Medikinet XL is an even split, so more arrives at once and it finishes sooner. Equasym XL sits between them.

Xaggitin XL and Delmosart are bioequivalent to Concerta XL, meaning they are designed to deliver the drug in the same way. They are still prescribed by brand.

Concerta XL side effects

Like all medicines, Concerta XL can cause side effects, and not everyone gets them. Most are mild, appear early, and settle within the first few weeks. Our general guide to ADHD medication side effects covers all the licensed options.

Common

  • Reduced appetite, and sometimes weight loss
  • Difficulty getting to sleep
  • Headache
  • Dry mouth
  • Stomach ache or nausea
  • Feeling anxious, restless or irritable
  • A modest rise in heart rate or blood pressure

Less common

  • Low mood, or mood becoming more changeable
  • Tics, or existing tics becoming more noticeable
  • Slowed growth in children, which is why height and weight are monitored
  • Palpitations

Because cover lasts into the evening, difficulty sleeping is a more common complaint with a twelve-hour product than with a shorter one, and is often addressed by taking the dose earlier.

When to contact your prescriber

Seek medical advice promptly for chest pain, breathlessness, fainting, a fast or irregular heartbeat, signs of poor circulation in the fingers or toes, or new or worsening thoughts of self-harm. Seek advice urgently if reality starts to feel distorted, for example hearing or seeing things that are not there, believing things other people do not, or becoming unusually suspicious; or if mood becomes unusually elevated, racing or overactive with little need for sleep. Do not stop the medication abruptly without advice. Suspected side effects can also be reported through the MHRA Yellow Card scheme.

Common questions

What does the XL in Concerta XL mean?

It indicates the modified-release formulation. Concerta XL releases 22 per cent of the dose immediately and the remaining 78 per cent gradually, giving up to twelve hours of cover from one morning dose.

Why can I see the tablet in the toilet?

The OROS shell passes through unchanged after the medicine inside has been released. This is expected and does not mean the dose was not absorbed.

Can Concerta XL be crushed or split?

No. It must be swallowed whole. Crushing, splitting or chewing destroys the release mechanism and delivers the full dose at once.

Concerta XL or Ritalin, which is better?

Neither is stronger. They contain the same drug. Concerta XL gives twelve hours from one dose; Ritalin gives three to four hours and finer control over timing.

Is Concerta XL a controlled drug?

Yes. It is Schedule 2, so it is treated differently from ordinary medicines rather than issued as a repeat prescription, each prescription covers about a month, and identification may be requested at the pharmacy.

Are Xaggitin XL and Delmosart the same as Concerta XL?

They are bioequivalent to it, meaning they are designed to deliver methylphenidate in the same way. They are still prescribed by brand rather than interchangeably.
