What Equasym XL is
Equasym XL is a modified-release brand of methylphenidate, taken once in the morning and lasting around eight hours. It uses a 30/70 split: 30 per cent of the dose is released immediately and 70 per cent gradually.
That profile is deliberately shaped around a school day. Eight hours of cover starting first thing takes a child through lessons without running late into the evening, which matters because a stimulant still active at bedtime is the most common cause of difficulty getting to sleep.
It sits between the immediate-release products and the twelve-hour ones. Ritalin lasts three to four hours; Concerta XL lasts up to twelve.
Why it is taken before breakfast
Equasym XL is taken before breakfast, not with it. This is the opposite instruction to Medikinet XL, which is taken with or just after food, and confusing the two is a genuine source of problems.
Taking it before eating means the immediate portion is absorbed promptly, so cover begins early in the morning rather than being delayed. Capsules can be opened and the contents sprinkled onto a small amount of soft food such as apple sauce and taken immediately without chewing, which helps where swallowing a capsule is difficult.
Two brands, two opposite instructions
Equasym XL is taken before breakfast. Medikinet XL is taken with or just after it. Both last around eight hours and both contain methylphenidate, which is exactly why the MHRA advises prescribing modified-release methylphenidate by brand. If your supply changes brand, check the instructions rather than assuming they carry over.
Who Equasym XL is prescribed for
Equasym XL is licensed in the UK for children aged six and over as part of a wider treatment programme. Unlike some other modified-release brands it does not hold an adult licence: its product information states it should not be used in adults for ADHD, because safety and efficacy have not been established in that age group. Adult use would therefore be off-label, and adults are normally prescribed Concerta XL or Medikinet XL instead.
NICE guideline NG87 makes methylphenidate the usual first choice for children and young people aged five and over.
An eight-hour product suits a school day or a standard working day. It is frequently chosen for children who need cover for lessons but not for the evening, and for anyone sensitive to a stimulant still being active at bedtime.
Clinical context
Checks before starting
A specialist will check height, weight, pulse and blood pressure before starting, and ask about heart conditions in you and your family. These are repeated during treatment, and in children height and weight are tracked at every review because stimulants can slow growth.
Methylphenidate is generally avoided where there is significant cardiovascular disease, uncontrolled high blood pressure, hyperthyroidism, glaucoma or phaeochromocytoma, and it is not combined with monoamine oxidase inhibitors.
Dosage and duration
Equasym XL is taken once daily before breakfast, and the dose is adjusted in steps at intervals of about a week with a review at each stage. Some people are established on it with a small immediate-release dose later in the day to extend cover, which is a specialist decision.
Cover typically lasts around eight hours. If it consistently runs out well before the day does, that is worth reporting: it is often addressed by adjusting the dose or moving to a longer-acting brand rather than by changing drug. Our guide to titration explains how those adjustments are made.
Controlled drug
Methylphenidate is Schedule 2, so it is treated differently from ordinary medicines rather than issued as a repeat prescription. Each prescription covers about a month, and identification may be requested at the pharmacy. Once your dose is stable, prescribing can sometimes move to your GP under a shared care agreement, although a practice can decline.
How Equasym XL compares
Eight hours, front-loaded less than Medikinet XL and taken differently.
Scroll the table sideways to see every column.
Equasym XL and Medikinet XL last a similar time but feel different, because a 30/70 split delivers less at once than a 50/50 one. People who find Medikinet XL comes on too sharply sometimes do better on Equasym XL at an equivalent total dose.
Equasym XL side effects
Like all medicines, this can cause side effects, and not everyone gets them. Most are mild, appear early, and settle within the first few weeks. Our general guide to ADHD medication side effects covers all the licensed options.
Common
- Reduced appetite, and sometimes weight loss
- Difficulty getting to sleep
- Headache
- Dry mouth
- Stomach ache or nausea
- Feeling anxious, restless or irritable
- A modest rise in heart rate or blood pressure
Less common
- Low mood, or mood becoming more changeable
- Tics, or existing tics becoming more noticeable
- Slowed growth in children, which is why height and weight are monitored
- Palpitations
When to contact your prescriber
Seek medical advice promptly for chest pain, breathlessness, fainting, a fast or irregular heartbeat, signs of poor circulation in the fingers or toes, or new or worsening thoughts of self-harm. Seek advice urgently if reality starts to feel distorted, for example hearing or seeing things that are not there, believing things other people do not, or becoming unusually suspicious; or if mood becomes unusually elevated, racing or overactive with little need for sleep. Do not stop the medication abruptly without advice. Suspected side effects can also be reported through the MHRA Yellow Card scheme.

Common questions
When should Equasym XL be taken?
Before breakfast. This differs from Medikinet XL, which is taken with or just after food, so it is worth checking the instructions for the specific brand you have.
How long does Equasym XL last?
About eight hours, which typically covers a school day or a standard working day without running late into the evening.
Can Equasym XL capsules be opened?
Yes. The contents can be sprinkled onto a small amount of soft food and taken immediately without chewing.
Is Equasym XL the same as Medikinet XL?
Both contain methylphenidate and both last around eight hours, but they use different release splits and opposite food instructions. They are not interchangeable.
Is Equasym XL a controlled drug?
Yes. It is Schedule 2, so it is treated differently from ordinary medicines rather than issued as a repeat prescription, each prescription covers about a month, and identification may be requested at the pharmacy.
