What Medikinet is

Medikinet is a brand of methylphenidate sold in the UK in two forms. Plain Medikinet is an immediate-release tablet lasting three to four hours. Medikinet XL is a modified-release capsule lasting around eight hours from a single morning dose.

Medikinet XL uses a 50/50 split: half the dose is released immediately and half gradually. That is a more front-loaded profile than Concerta XL, which releases 22 per cent immediately, and it is why Medikinet XL comes on more noticeably but finishes sooner.

Both forms work in the same way as any methylphenidate, by increasing the availability of dopamine and noradrenaline in the parts of the brain that handle attention, motivation and impulse control.

Why Medikinet XL is taken with food

This is the single most important practical point about Medikinet XL, and it is frequently missed. The capsule is designed to be taken with or just after breakfast, not on an empty stomach.

Food changes how the modified-release portion is absorbed. Taken without it, the release profile is altered and the medicine may not deliver the cover it is designed to give. People who report that Medikinet XL simply stopped working partway through the morning have often changed their breakfast habits without realising the connection.

A practical consequence

If breakfast is unreliable, Medikinet XL may not be the right choice. That is worth raising honestly at review rather than trying to force a routine that does not fit, because Concerta XL and Equasym XL have different requirements.

Capsules can be opened and the contents sprinkled onto a small amount of soft food such as yoghurt or apple sauce and taken immediately, without chewing. That helps where swallowing a capsule is difficult, and it still counts as taking it with food.

Who Medikinet is prescribed for

The two products are licensed differently, which matters for adults. Medikinet XL is licensed for children aged six and over and for adults. Immediate-release Medikinet is licensed for children only, so prescribing it to an adult is off-label and a specialist decision. That is the position for short-acting stimulant medication generally in the UK: it is not licensed for adults, and the modified-release products are.

NICE guideline NG87 makes methylphenidate the usual first choice for children and young people, and one of two first-line options for adults.

An eight-hour product suits a school day or a standard working day without running late into the evening. People who are sensitive to a stimulant still being active at bedtime often do better on Medikinet XL than on a twelve-hour option.

Clinical context

Checks before starting

A specialist will check height, weight, pulse and blood pressure before starting, and ask about heart conditions in you and your family. These checks are repeated during treatment, and in children height and weight are tracked at every review.

Methylphenidate is generally avoided where there is significant cardiovascular disease, uncontrolled high blood pressure, hyperthyroidism, glaucoma or phaeochromocytoma, and it is not combined with monoamine oxidase inhibitors.

Dosage and how it is taken

Medikinet XL is taken once daily with or just after breakfast, and the dose is adjusted in steps at intervals of about a week with a review at each stage. Immediate-release Medikinet is taken two or three times a day.

Some people are established on Medikinet XL with a small immediate-release dose later in the day to extend cover. That is a specialist decision rather than something to arrange independently. Our guide to titration explains how the dose is worked out.

Prescribed by brand

The MHRA advises that modified-release methylphenidate is prescribed by brand name. Concerta XL, Medikinet XL and Equasym XL release the dose differently and are not interchangeable. If your supply changes brand, raise it rather than assuming it makes no difference.

Controlled drug

Methylphenidate is Schedule 2, so it is treated differently from ordinary medicines rather than issued as a repeat prescription. Each prescription covers about a month, and identification may be requested at the pharmacy. Once your dose is stable, prescribing can sometimes move to your GP under a shared care agreement, although a practice can decline.

How Medikinet XL compares

Between the immediate-release options and the twelve-hour ones.

Brand Release Duration Notes
Medikinet XL Modified, 50/50 About 8 hours With or just after breakfast
Equasym XL Modified, 30/70 About 8 hours Before breakfast
Concerta XL Modified, OROS 22/78 Up to 12 hours With or without food
Ritalin Immediate 3 to 4 hours Two or three doses a day

Scroll the table sideways to see every column.

Medikinet XL and Equasym XL both last around eight hours but differ in when they are taken and how the dose is split. Equasym XL is 30/70 and taken before breakfast; Medikinet XL is 50/50 and taken with it. Those are not interchangeable instructions.

Medikinet side effects

Like all medicines, this can cause side effects, and not everyone gets them. Most are mild, appear early, and settle within the first few weeks. Our general guide to ADHD medication side effects covers all the licensed options.

Common

  • Reduced appetite, and sometimes weight loss
  • Difficulty getting to sleep
  • Headache
  • Dry mouth
  • Stomach ache or nausea
  • Feeling anxious, restless or irritable
  • A modest rise in heart rate or blood pressure

Less common

  • Low mood, or mood becoming more changeable
  • Tics, or existing tics becoming more noticeable
  • Slowed growth in children, which is why height and weight are monitored
  • Palpitations

Because Medikinet XL is more front-loaded than Concerta XL, some people notice the onset more distinctly. Where that is uncomfortable, a less front-loaded formulation is sometimes a better fit.

When to contact your prescriber

Seek medical advice promptly for chest pain, breathlessness, fainting, a fast or irregular heartbeat, signs of poor circulation in the fingers or toes, or new or worsening thoughts of self-harm. Seek advice urgently if reality starts to feel distorted, for example hearing or seeing things that are not there, believing things other people do not, or becoming unusually suspicious; or if mood becomes unusually elevated, racing or overactive with little need for sleep. Do not stop the medication abruptly without advice. Suspected side effects can also be reported through the MHRA Yellow Card scheme.

Common questions

What is the difference between Medikinet and Medikinet XL?

Medikinet is immediate release and lasts three to four hours. Medikinet XL is modified release, lasts around eight hours from one morning dose, and is taken with food.

Does Medikinet XL have to be taken with food?

Yes. It is designed to be taken with or just after breakfast. Taken on an empty stomach the release profile is altered and it may not give the cover intended.

Can Medikinet XL capsules be opened?

Yes. The contents can be sprinkled onto a small amount of soft food and taken immediately without chewing, which still counts as taking it with food.

How long does Medikinet XL last?

About eight hours, which suits a school day or standard working day without running late into the evening.

Is Medikinet the same as Concerta XL?

Both contain methylphenidate but they release it differently and are not interchangeable. The MHRA advises prescribing modified-release methylphenidate by brand.

Is Medikinet a controlled drug?

Yes. It is Schedule 2, so it is treated differently from ordinary medicines rather than issued as a repeat prescription, each prescription covers about a month, and identification may be requested at the pharmacy.
