What Elvanse is

Elvanse is the UK brand name for lisdexamfetamine, one of the stimulants used to treat ADHD. It belongs to the same family as the other options covered in our guide to ADHD medication in the UK, and it is among the most widely prescribed for adults.

What makes it unusual is that it is a prodrug. The capsule you swallow is inactive. Once absorbed, the body gradually converts it into dexamfetamine, which is the part that acts on the brain. Because that conversion happens steadily, the effect builds gently and tapers off gradually, without the sharper peaks and troughs some people notice with immediate-release stimulants.

Elvanse and Vyvanse are the same medication. Vyvanse is the name used in the United States and some other countries. If you have read about Vyvanse online the clinical information applies, but UK licensing, dosing conventions and prescribing rules are the ones set out here.

Who it is prescribed for

In the UK, Elvanse is licensed for children aged six and over as part of a wider treatment programme, and Elvanse Adult is licensed for adults. It is not licensed for children under six. Where an adult was already established on it in childhood, treatment can continue into adulthood under specialist review.

NICE guideline NG87 sets out the UK position. For adults, either
lisdexamfetamine or methylphenidate is offered first. For children and young people aged five and over, methylphenidate is the usual first choice, with lisdexamfetamine considered where a six-week trial at an adequate dose has not helped enough.

Clinical context

Medication is one part of treatment, not the whole of it

NICE is clear that medication should never be the only support offered. It sits alongside education about ADHD, adjustments at work or school, and coaching and therapy. Many people find the combination more useful than either alone.

Before any stimulant is started, a specialist checks height, weight, pulse and blood pressure, and asks about heart conditions in you and your family. These checks are repeated during treatment.

Elvanse is not suitable for everyone. It is generally avoided where there is significant cardiovascular disease, uncontrolled high blood pressure, hyperthyroidism, glaucoma, or a history of certain psychiatric conditions, and it is not combined with monoamine oxidase inhibitors. A full assessment is how those questions get answered properly, which is why the decision sits with a Consultant Psychiatrist rather than a symptom checklist.

Dosage and titration

Elvanse comes as capsules from 20mg to 70mg. Treatment usually starts at 30mg once daily in the morning, though 20mg may be chosen where a more cautious approach is sensible. The dose is then adjusted in steps of 10mg or 20mg, at intervals of about a week.

This process of finding the right dose is called titration. Our guide to titration explains what the appointments involve and how long it usually takes. The aim is the lowest dose that gives a useful benefit with side effects you find acceptable, not the highest dose you can tolerate.

Capsules are swallowed whole, or opened and the contents mixed into water or a soft food and taken straight away. Taking it at a consistent time each morning matters more than most people expect, because a late dose is the most common reason for difficulty getting to sleep.

Controlled drug

Lisdexamfetamine is Schedule 2, so it is treated differently from ordinary medicines rather than issued as a repeat prescription, each prescription covers about a month, and identification may be requested at the pharmacy. Once your dose is stable, prescribing can sometimes move to your GP under a shared care agreement,
although a practice can decline.

How long it lasts

Elvanse is designed to last a long day. Most people notice an effect within about an hour to ninety minutes, with cover typically extending up to 13 to 14 hours from the morning dose. That is longer than most other stimulant options available here, which is one reason it suits adults with long working days.

In practice a typical day might look like this: the capsule at around seven in the morning, the effect building through the first hour, holding steadily through the working day, and easing off during the evening. Some people notice a period as it wears off where tiredness or low mood is more apparent. If that happens consistently it is worth raising, because it is often addressed by adjusting the dose or the timing rather than changing medication.

Duration varies between individuals. A dose that covers one person until early evening may fade by mid-afternoon for someone else. Keeping a simple record of when the effect starts and ends during titration gives your prescriber something concrete to work with.

Side effects

Like all medicines, Elvanse can cause side effects, and not everyone gets them. Most are mild, appear early, and settle within the first few weeks. Our general guide to ADHD medication side effects covers what to watch for across all the licensed options.

Common

  • Reduced appetite, and sometimes weight loss
  • Difficulty getting to sleep
  • Dry mouth
  • Headache
  • Feeling anxious, restless or irritable
  • Nausea or stomach discomfort
  • A modest rise in heart rate or blood pressure

Less common

  • Low mood, or mood becoming more changeable
  • Tics, or existing tics becoming more noticeable
  • Slowed growth in children, which is why height and weight are monitored
  • Palpitations

When to contact your prescriber

Seek medical advice promptly for chest pain, breathlessness, fainting, a fast or irregular heartbeat, signs of poor circulation in the fingers or toes, or new or worsening thoughts of self-harm. Seek advice urgently if reality starts to feel distorted, for example hearing or seeing things that are not there, believing things other people do not, or becoming unusually suspicious; or if mood becomes unusually elevated, racing or overactive with little need for sleep. Do not stop the medication abruptly without advice. Suspected side effects can also be reported through the MHRA Yellow Card scheme.

How it compares

The stimulants licensed in the UK differ mainly in how long they last and how the dose is released across the day.

Common questions

Is Elvanse the same as Vyvanse?

Yes. They are the same medication, lisdexamfetamine. Vyvanse is the brand name used in the United States and several other countries, and Elvanse is the name used in the UK. Only the UK brands can be prescribed and dispensed here.

Is Elvanse used for binge eating disorder?

In the United States, lisdexamfetamine is approved for moderate to severe binge eating disorder in adults, where it is sold as Vyvanse. That licence does not apply in the UK, where lisdexamfetamine is licensed for ADHD only, so prescribing it for binge eating here would be off-label. Even in the US it is not approved for weight loss.

Can my GP prescribe Elvanse?

A GP cannot start ADHD medication. Treatment has to be initiated by a specialist, such as a Consultant Psychiatrist, after a full assessment. Once your dose is stable, a GP may agree to continue prescribing under a shared care agreement, though practices can decline. Our guide to getting ADHD medication sets out the full pathway.

How long does Elvanse take to work?

Most people notice an effect within about an hour to ninety minutes of the morning dose. Finding the dose that works well for you is a separate question and usually takes several weeks of titration.

Is Elvanse a controlled drug?

Yes. Lisdexamfetamine is a Schedule 2 controlled drug in the UK. In practice that means it is treated differently from ordinary medicines rather than issued as a repeat prescription, each prescription covers about a month, and identification may be requested at the pharmacy.

What happens if I miss a dose?

If you remember later the same morning, you can usually take it then. If it is already afternoon, it is generally better to skip that day rather than risk being awake late into the night. Never take two doses to make up for a missed one, and check with your prescriber or pharmacist if you are unsure.

Can I drink alcohol while taking Elvanse?

The Elvanse product information states only that there are limited data on the possible interaction with alcohol, which is an absence of evidence rather than reassurance. Alcohol can also mask how intoxicated you feel on a stimulant. Raise it with your prescriber so the advice fits your circumstances.

Important

This information is general and educational. It is not a substitute for medical advice. Decisions about starting, changing or stopping ADHD medication are always made together with a Consultant Psychiatrist following a full assessment.
